  • Improved awareness of when to discharge a patient home with hospice versus continuing care of the dying patient in the hospital 

  • Understand the Evidence Based and Best Practices for Care of the Dying Patient through use of the CARES Tools

  • Increase awareness of the limits of the Comfort Order Set and when to consult supportive medicine
